equity to pediatric healthcare by offering underserved patients direct access to their primary care
provider through traditional medical office visits, telemedicine, and mobile medical services while
providing comprehensive, prevention-oriented healthcare, care navigation services, and integrated
behavioral health services.
About the Wellness Coordinator
BCP is currently seeking a Wellness Coordinator to join our team. The Wellness Coordinator will work closely with our Care Navigators while supporting the wellness related needs of our patients. This position will oversee the planning and executing of programming to support patients; including cooking classes, group exercise classes, BCP’s tutoring program, reading program, Girls on the Run, summer programming and supermarket tours, etc., and assisting patients in acquiring health and wellness supplies. BCP’s cooking class is one of BCP’s most popular programs and the wellness coordinator will be responsible for organizing the delivery of food to patient homes prior to the cooking class, developing recipes and teaching the weekly class.
The Wellness Coordinator will be the primary contact for BCP’s Healthy in the City partnership with Boston Children’s Hospital, identifying and enrolling families into the program during appointments, to set goals and support patients in participating in fitness and nutrition activities. The coordinator will facilitate physical activity program sign ups with the YMCA and other external programs, such as soccer and basketball. They will also track participation in our programs and support in the preparation of our quarterly grant report to Boston Children’s Hospital.
Additionally the wellness coordinator will play a large role in the coordination and execution of BCP’s
special events for patients, including Camp Night, Job Night, Back to School Night, Thanksgiving Turkey
& Dinner Distribution and BCP’s Holiday Shop.
Integrated Care Navigation and Wellness services are a critical component of BCP’s successful model in centering patients and their needs in all that we do as we work to improve the way healthcare is delivered to all children. The Wellness Coordinator will report to the Director of Care Navigation and Wellness and will be an important team member of the Care Navigation and Wellness Department.
Key Responsibilities:

Coordinate and run on-sight and virtual after-school activities, including dance, fitness, reading and Girls on the Run (operates Spring and Fall)

Coordinate and support additional wellness-related activities at the practice, including the Well Child group, Baby Café (not yet started), support groups, youth groups, etc.

Facilitate BCP Nutritious, our weekly cooking class, including ordering and ensuring delivery of all ingredients ahead of the weekly cooking class and teaching the class onsite and over zoom

Track and report attendance for each of our programming activities

Provide ongoing outreach and goal setting with patients to reinforce engagement in fitness and nutrition activities

Deliver healthy food/physical activity resources to families, including fresh fruits/vegetables, frozen meals, grocery store gift cards, and exercise equipment

Develop innovative and creative ways to serve patients' wellness-related needs

Gather/develop educational materials to support patients

Collaborate with all providers at BCP

Collaborate with community organizations to refer patients to programming

Recruit patients for programs in conjunction with providers and BCP staff
-
Participate in various special events throughout the year in conjunction with BCP staff: camp night, job night, back to school event, Thanksgiving turkey distribution and Holiday store including outreach and engagement with families
Requirements

Bachelor's degree in a related field (nutrition, education, public health, etc.)m

2+ years working with children preferred

Openness to and respect for others’ cultures, identities, and lived experiences

Organized with the ability to effectively prioritize responsibilities/tasks in a fast-paced environment

Detail-oriented and able to complete work accurately and efficiently

Excellent written and verbal communication skills
